Before your appointment. Please download and fill out a registration form from our website. You can save and e-mail it to us prior to your appointment. If not, please arrive 15 minutes early, and you can fill out the forms in our office. Please make sure to bring your driver’s license, insurance card if you intend to use dental coverage, and your social security number for our records.

Even cleanings are performed by Dr. Gotun himself to personally monitor his patients' progress. We offer a full spectrum of services in our practice, including orhtodontics and implants, so there is no need to refer you to another specialist, unless some exceptional circumstances arise. We practice quadrant dentistry ( when possible we work on a quarter of your mouth at a time to save you time and money).
